Faithful Insurance Agency LLC ("Agency," "we," "us," or "our") respects your privacy. This Privacy Policy explains how we collect, use, disclose, protect, and retain information submitted through our website and online insurance application.

When you voluntarily submit information through our online insurance application, we may collect information such as:

If you provide your telephone number and provide the consent requested on our online application, we may contact you by telephone call or text message consistent with the consent you provided and applicable federal and state law.

Your consent is not a condition of purchasing insurance.

You may request that we stop sending marketing or solicitation communications by contacting us directly. For text messages, you may also reply STOP where that functionality is available.

Withdrawal of marketing consent does not necessarily prevent communications that are necessary to service an existing insurance relationship or complete a transaction.

We may use third-party technology and service providers to operate our website, receive form submissions, communicate with consumers, maintain records, obtain insurance quotations, and conduct insurance transactions.

Formspree. Our website uses Formspree as a third-party form-processing service to receive and route information submitted through our online application. Formspree processes information on our behalf as part of our website and form-submission workflow. We do not authorize Formspree to independently market our submitted application information to unrelated third-party telemarketers.

Smart Choice and Insurance Companies. When necessary to respond to your insurance inquiry, we may provide relevant information to Smart Choice, participating insurance companies, managing general agencies, wholesalers, or other insurance-marketplace participants with whom our Agency works. Information may be used to determine eligibility, obtain insurance quotations, evaluate risks, prepare applications, and facilitate an insurance transaction. Not every website submission is necessarily sent to every carrier or marketplace. Information is shared only as reasonably necessary for the insurance services you request, subject to applicable law and our contractual obligations.

5. No Sale or Rental of Consumer Information

We do not sell or rent consumer inquiry information to unrelated third-party telemarketing companies for their independent marketing purposes. This does not prevent disclosures to service providers, insurance companies, insurance marketplaces, regulators, legal advisers, or other parties when reasonably necessary to provide requested insurance services, maintain our business, comply with law, prevent fraud, or complete an insurance transaction.

Insurance information may be subject to federal and state privacy requirements, including applicable provisions of the Gramm-Leach-Bliley Act and state insurance privacy laws. Where applicable, we maintain administrative, technical, and physical safeguards designed to protect nonpublic personal information against unauthorized access, use, disclosure, alteration, or destruction.

No electronic transmission or storage system can be guaranteed to be completely secure.

We retain records for periods reasonably necessary to conduct our business and satisfy applicable legal, regulatory, contractual, insurance, and accounting requirements. For insurance transactions subject to applicable Illinois recordkeeping requirements, records may be retained for seven years following completion of the insurance transaction as required by applicable Illinois regulations (50 Ill. Admin. Code § 3120.80), which addresses information collected from consumers, disclosures, and information used in recommendations forming the basis for insurance transactions.
