Privacy Policy
Faithful Insurance Agency LLC ("Agency," "FiA," "us," or "team") respects privacy. This Privacy Policy explains how FiA collects, utilizes, discloses, protects, and retains information submitted through the FiA website and online insurance application.
1. Information FiA Collects
When information is voluntarily shared through the agency online insurance application, FiA may collect:
- Name
- Telephone Number
- Email Address
- Insurance-related information provided in response to the FiA application questions
- Current insurance status or coverage interests
- Information necessary to understand a request and obtain an insurance quotation
- Technical information associated with the website submission, such as submission date and time and information generated by the website or form-processing service
FiA collects only the information reasonably necessary to respond to an insurance inquiry, communicate with clients, evaluate client insurance needs, obtain quotations, and provide insurance services.
2. How FiA Utilizes Information
- Respond to client inquiries
- Contact clients regarding the insurance products or services about which information was requested
- Understand client insurance needs
- Obtain quotations or underwriting information from insurance companies or insurance-marketplace partners
- Prepare or assist with an insurance application
- Communicate with clients regarding an existing or prospective insurance relationship
- Maintain records required by law or applicable insurance requirements
- Detect, investigate, and prevent fraud, misutilization, unauthorized access, or security incidents
- Comply with legal and regulatory requirements
3. Telephone and Text Communications
If a client provides a telephone number and the consent requested on the FiA online application, FiA may contact the client by telephone call or text message consistent with the client consent provided and applicable federal and state law.
Client consent is not a condition of purchasing insurance.
Clients may request that FiA stop sending marketing or solicitation communications by contacting us directly. For text messages, you may also reply STOP where that functionality is available.
Withdrawal of marketing consent does not necessarily prevent communications that are necessary to service an existing insurance relationship or complete a transaction.
4. Third-Party Service Providers
FiA may utilize third-party technology and service providers to operate the FiA website, receive form submissions, communicate with consumers, maintain records, obtain insurance quotations, and conduct insurance transactions.
Formspree. The FiA website utilizes Formspree as a third-party form-processing service to receive and route information submitted through the online application. Formspree processes information for FiA as part of the FiA website and form-submission workflow. FiA does not authorize Formspree to independently market any submitted application information to unrelated third-party telemarketers.
Smart Choice and Insurance Companies. When necessary to respond to client insurance inquiry, FiA may provide relevant information to Smart Choice, participating insurance companies, managing general agencies, wholesalers, or other insurance-marketplace participants with whom the team works. Information may be utilized to determine eligibility, obtain insurance quotations, evaluate risks, prepare applications, and facilitate an insurance transaction. Not every website submission is necessarily sent to every carrier or marketplace. Information is shared only as reasonably necessary for the insurance services a client requests, subject to applicable law and FiA contractual obligations.
5. No Sale or Rental of Consumer Information
FiA does not sell or rent consumer inquiry information to unrelated third-party telemarketing companies for any independent marketing purposes. This does not prevent disclosures to service providers, insurance companies, insurance marketplaces, regulators, legal advisers, or other parties when reasonably necessary to provide requested insurance services, maintain any FiA-related business, comply with law, prevent fraud, or complete an insurance transaction.
6. Insurance and Financial Privacy
Insurance information may be subject to federal and state privacy requirements, including applicable provisions of the Gramm-Leach-Bliley Act and state insurance privacy laws. Where applicable, FiA maintains administrative, technical, and physical safeguards designed to protect nonpublic personal information against unauthorized access, utilization, disclosure, alteration, or destruction.
7. Information Security
FiA maintains written information-security procedures appropriate to the nature and sensitivity of information handled by the Agency. Depending on the system involved, safeguards may include multi-factor authentication, password and access controls, device security, encryption, restricted access, vendor security controls, security monitoring, data backup and recovery procedures, and incident-response procedures. No electronic transmission or storage system can be guaranteed to be completely secure.
8. Record Retention
FiA retains records for periods reasonably necessary to conduct any FiA-related business and satisfy applicable legal, regulatory, contractual, insurance, and accounting requirements. For insurance transactions subject to applicable Illinois recordkeeping requirements, records may be retained for seven years following completion of the insurance transaction as required by applicable Illinois regulations (50 Ill. Admin. Code § 3120.80), which addresses information collected from consumers, disclosures, and information utilized in recommendations forming the basis for insurance transactions.
9. Cookies and Website Technologies
The FiA website may utilize cookies, analytics, security technologies, or similar technologies necessary to operate, secure, maintain, and improve the website.
10. Children's Information
The FiA website is intended for adults seeking insurance information and services. FiA does not solicit personal information from children under 13.
11. Changes to This Policy
FiA may update this Privacy Policy from time to time to reflect changes in business, technology, legal requirements, or privacy practices. The effective date at the beginning of this policy identifies the current version.
